The three components

The GCS assesses eye opening (E, 1–4), verbal response (V, 1–5) and motor response (M, 1–6). Always record the components, not just the total — “GCS 14 (E4 V4 M6)” — as the motor score carries the most prognostic weight and the same total can mean different things.

GCS is the sum of three components: best eye opening (1–4), best verbal response (1–5), and best motor response (1–6). The total ranges from 3 (deep coma) to 15 (fully alert).

What GCS is considered severe?+

A total of 13–15 is mild, 9–12 is moderate, and 8 or below is severe. A GCS ≤ 8 is a common threshold for considering airway protection (intubation).

How should GCS be documented?+
