How weight-based dosing works

Most pediatric medicines are dosed by body weight. The dose is the child’s weight in kilograms multiplied by a drug-specific rate in mg/kg. Rates may be written per dose or per day — always confirm which the reference intends, then divide a daily total by the number of doses.

Worked example

A 12 kg child prescribed amoxicillin 15 mg/kg/dose, three times a day: 12 × 15 = 180 mg per dose(540 mg/day). With a 250 mg/5 mL (50 mg/mL) suspension, that is 180 ÷ 50 = 3.6 mL per dose.

How is a weight-based pediatric dose calculated?+

Multiply the child’s weight in kilograms by the prescribed dose in mg/kg. For example, 12 kg × 15 mg/kg = 180 mg per dose. If the rate is expressed per day, divide the daily total by the number of doses to get each dose.

What is the difference between mg/kg/dose and mg/kg/day?+

mg/kg/dose is the amount for a single administration; mg/kg/day is the total for 24 hours, split across the daily frequency. This calculator lets you choose either basis so the maths is correct.

How do I convert a dose to a liquid volume?+

Divide the dose (mg) by the syrup/suspension concentration (mg/mL). Enter the concentration and the tool shows the millilitres to draw up.

Does it apply a maximum dose?+
